Periodic Health Assessments (PHA) and/or Mental Health Assessment (MHA)

To initiate your MHA/PHA, please click the PHAQ Link located in the PHA Column of your IMR Page. Please note: If you are not due for your PHA, you will be prohibited to start a new PHAQ without the MTF unlocking it. Once you complete the PHAQ, you will need to do one of the following:

For PHAQ Priority Responses and members who are on AUoF/Fly status, please contact the appointment line at 1-866-DRS-APPT (1-866-377-2778) to schedule your virtual MHA/PHA with the 87th MDG.

For all routine PHA/MHA’s please contact the Virtually Integrated Patient Readiness and Remote (VIPRR) Care Clinic from a personal cell phone at 1-844-VMEDCEN (1-844-863-3236).

Medical Clearance

In ASIMS, select the “New Medical Clearance Request”, and choose appropriate type of request being submitted. After submitting your request, you can check your status in your IMR. Answer the medical questionnaire within the application and then send all supporting documents to the med standards org box dha.jbmdl.mcguire-87th-mdg.mbx.87-omrs-med-standards@health.mil
NOTE: If you initiated a clearance through your MyIMR, and do not complete/submit supporting documentation within 72 hours, the system will automatically delete your request and you will need to re-submit your request.

Deployment Health Assessments (DRHA/DHA)

You are due for a DRHA/DHA if you have recently been tasked with a new deployment or have returned from a deployment. Once you've coordinated with Public Health and received your Deployment Checklist, complete the DRHA is the online questionnaire, and the DHA is the appointment with the MTF provider.

DHA 1 & 2 require a face-to-face appointment. Complete the online form in MyIMR and call 1-866-DRS-APPT (1-866-377-2778) to schedule your face-to-face appointment. If you are scheduled and your questionnaire is not accomplished prior to your appointment, it will need to be cancelled.

All members with any outstanding/open/pending referrals cannot be cleared until after these consults have been reviewed. Outside records must be faxed to the 87th MDG 609-754-9249 and should include PCM/provider assessment, plan of care, and next follow up or follow up "as needed" and forward to 87th MDG for PCM review.
Force Health Protection Prescription Products (FHPPPs)

FHPPPs are comprised of such products as atropine and/or 2-Pam chloride auto-injectors, certain antimicrobials, anti-malarials, and pyridostigmine bromide.

Provider will order your medication at your DHA 1 appointment.

Separation Health and Physical Examination (SHPE)
Who needs a SHPE?
- Service members on active duty for 180 days or more of continuous service
- SM on active duty approved for Palace Chase/Palace Front transition
- SM on active duty approved for SkillBridge (Can use final out date)
- National Guard or Reserve on AD orders for 180 days or more of continuous service
- National Guard or Reserve on AD orders for 31 days or more in support of a contingency operation
- Initiate SHPE physical by emailing org box: dha.jbmdl.McGuire-87th-MDG.mbx.87-mdg-sepret-physical@health.mil
Who doesn’t need a SHPE?
- The SM is separating or retiring after being found unfit and has been referred to the Disability Evaluation System
- NOTE: Initial Review in Lieu of or referral to a Medical Evaluation Board doesn’t negate the SHPE requirement. MTF staff must wait until the IRILO or MEB decision is returned to determine if SHPE is required.
SHPE Timeline
- GREATER than 180 days before separation/retirement:
- Ensure all off-base/specialty care documents are sent to MTF
- SM going through Benefit Delivery at Discharge should contact the VA benefits advisor at McGuire AFB by calling 571-461-8702
- 180-90 days before separation/retirement:
- Follow MTF/VA instructions below
- Less than 90 days before separation/retirement: SHPE must be completed at MTF
- Initiate DD Form 2807-1, Report of Medical History, by going to SHPE module and selecting ’Start New SHPE’ in MyIMR
- Schedule appointments with SHPE coordinator using the org box email above
Steps to completing SHPE at MTF: Completion must be less than 180 days before separation, terminal leave, or retirement, whichever is sooner.
- Initiate DD Form 2807-1, Report of Medical History, by going to SHPE module and selecting ’Start New SHPE’ in MyIMR
- Complete SHPE Mental Health Assessment on MyIMR
- Schedule and complete audiogram (must be within 6 months of separation date) call the appointment line at 1-866-DRS-APPT (1-866-377-2778)
- Complete any other Individual Medical Readiness requirements not currently green:
- PHA/MHA
- HIV lab
- Occupational Health termination exam (certain AFSCs only)
- SHPE Coordinator schedules face-to-face SHPE appointment and provides memo; bring it to the appointment for signature
- SHPE Coordinator verifies SHPE completion and signs vOP in vMPF
Steps to completing SHA (in lieu of SHPE) at the VA: Must be completed greater than 30 days before final out.
- Contact Military Service Coordinators at the AFRC
- Initiate DD Form 2807-1, Report of Medical History by going to SHPE module and selecting ’Start New SHPE’ in MyIMR
- Must be completed within 180 days from separation, terminal leave, or retirement, whichever is sooner
- Do not check the ‘VA SHPE’ box (Answer ‘no’ when prompted), regardless of whether you plan to file a claim or not. The system won’t allow the MHA portion to be completed with this box checked; it has no bearing on any VA claims made, since that’s a separate process.
- Schedule and complete audiogram (must be within 6 months of separation date) call the appointment line at 1-866-DRS-APPT (1-866-377-2778)
- Complete any other IMR requirements not currently green:
- PHA/MHA
- HIV lab
- Occupational Health termination exam (AD only)
- The Separation Health Assessment as part of the VA claims process can fulfill the SHPE requirement if documentation is placed in the SM’s electronic health record 30 days before separation. If unable to submit at least 30 days before separation date, a SHPE must be completed at the MTF.
- Come to Flight Medicine for SHPE Coordinator to verify SHA and SHPE memo; will sign vOP in vMPF.
