Base Operation Medical Cell (BOMC)

Periodic Health Assessments (PHA) and/or Mental Health Assessment (MHA)
  • To initiate your MHA/PHA, please click the PHAQ Link located in the PHA Column of your IMR Page. Please note: If you are not due for your PHA, you will be prohibited to start a new PHAQ without the MTF unlocking it. Once you complete the PHAQ, you will need to do one of the following:

  • For PHAQ Priority Responses and members who are on AUoF/Fly status, please contact the appointment line at 1-866-DRS-APPT (1-866-377-2778) to schedule your virtual MHA/PHA with the 87th MDG.

  • For all routine PHA/MHA’s please contact the Virtually Integrated Patient Readiness and Remote (VIPRR) Care Clinic from a personal cell phone at 1-844-VMEDCEN (1-844-863-3236).

  • For all GSU, IMA, Guard, Reserve, and Sister Service members please email the PHA org box dha.jbmdl.mcguire-87th-mdg.mbx.87-omrs-bomc-pha-cell@health.mil  with completed supporting documents to be scheduled:

    • DD Form 2870, Authorization for Disclosure of Medical or Dental Information. Fax 609-754-9249
    • 2807-1, Report of Medical History
    • GSU PHA Civilian Provider Template (Signed by Civilian PCM)
Medical Clearance
  • In ASIMS, select the “New Medical Clearance Request”, and choose appropriate type of request being submitted.  After submitting your request, you can check your status in your IMR.  Answer the medical questionnaire within the application and then send all supporting documents to the med standards org box dha.jbmdl.mcguire-87th-mdg.mbx.87-omrs-med-standards@health.mil
    NOTE: If you initiated a clearance through your MyIMR, and do not complete/submit supporting documentation within 72 hours, the system will automatically delete your request and you will need to re-submit your request. 

  • Due to high volume of calls please DO NOT CALL appointment line for status updates or contact staff directly. Check IMR or email the org box.

  • When sending supporting documentation to the org box *REMOVE ALL PII*

  • PLEASE DO NOT BRING DOCUMENTS TO MDG AND ONLY COMMUNICATE VIA ORG BOX.

    • Retraining/Re-class/Special Duty

      • Send your retraining myPers response email to the org box

    • Overseas Medical Clearance (AD only)

      • Send Medical Clearance Memo, RNLTD, PPC (on RIP), and EDD to org box.  

      • PCS clearances need to be submitted within 7 days of assignment.

    • Palace Chase/Front 422 Request

      • Complete the medical questions and provide a tentative separation date.

      • Send recruiters letter to the org box

      • Initiate Separation Health and Physical Examination (SHPE).

    • Initial Flying Class Physicals

    • Commissioning

      • Send CC letter and requirements for 422 from application package to org box

    • PME

      • Send class RIP or class email notification to org box

Deployment Health Assessments (DRHA/DHA)
  • You are due for a DRHA/DHA if you have recently been tasked with a new deployment or have returned from a deployment. Once you've coordinated with Public Health and received your Deployment Checklist, complete the DRHA is the online questionnaire, and the DHA is the appointment with the MTF provider.

    • DHA 1 & 2 require a face-to-face appointment. Complete the online form in MyIMR and call 1-866-DRS-APPT (1-866-377-2778) to schedule your face-to-face appointment. If you are scheduled and your questionnaire is not accomplished prior to your appointment, it will need to be cancelled.

    • ​For all GSU, IMA, Guard, Reserve, and Sister Service members please email the BOMC Deployment Readiness org box dha.jbmdl.McGuire-87th-MDG.mbx.87-omrs-bomc-deployment-readiness@health.mil completed supporting documents to be scheduled:

      • DD Form 2870, Authorization for Disclosure of Medical or Dental Information. Fax 609-754-9249

      • 2807-1, Report of Medical History

      • GSU PHA Civilian Provider Template (Signed by Civilian PCM)

    • DHA 3/4/5 will be accomplished via phone. AFTER completing your questionnaire, please email the following workbox with your name, DODI, which DRHA you're needing and phone number: usaf.jbmdl.87-mdg.mbx.87-omrs-bomc-deployment-readiness@health.mil

    • All members with any outstanding/open/pending referrals cannot be cleared until after these consults have been reviewed.  Outside records must be faxed to the 87th MDG 609-754-9249 and should include PCM/provider assessment, plan of care, and next follow up or follow up "as needed" and forward to 87th MDG for PCM review.

 Force Health Protection Prescription Products (FHPPPs)
  • FHPPPs are comprised of such products as atropine and/or 2-Pam chloride auto-injectors, certain antimicrobials, anti-malarials, and pyridostigmine bromide.

  • Provider will order your medication at your DHA 1 appointment.

  • All large group requests to include Sister Service/AGR/RC will be processed by emailing usaf.jbmdl.87-mdg.mbx.87-omrs-bomc-deployment-readiness@health.mil

  • Pick up from 87th MDG pharmacy prior to departure.

 Separation Health and Physical Examination (SHPE)
  • Who needs a SHPE?

    • Service Members (SM) on Active Duty (AD) for 180 days or more of continuous service.

    • SM on Active Duty (AD) approved for Palace Chase/Palace Front transition.

    • SM on Active Duty (AD) approved for SkillBridge. (Can use Final out date)

    • Reserve Component (RC) on AD orders for 180 days or more of continuous service.

    • RC on AD orders for 31 days or more in support of a contingency operation.

    • Initiate SHPE physical by contacting org box: dha.jbmdl.McGuire-87th-MDG.mbx.87-mdg-sepret-physical@health.mil.

  •  Who does not need a SHPE?
    • SM going through Benefit Delivery Discharge (BDD) contact VA benefits advisor at McGuire: 571-461-8702.

      • All still must complete Separation Health Assessment (SHA) as part of the VA claims process. SHA can fulfill the SHPE requirement if documentation is uploaded in the members electronic health record 30 days prior to separation. Please see “Steps to completing your SHA (in lieu of SHPE) at the VA.”

      • SM are still required to be signed off in Virtual Out-Processing (vOP).

  • SHPE Timeline

    • GREATER than 180 days prior to separation/retirement:

      • Ensure all off base/specialty care documents are sent to MTF.

    • 180-90 days prior to separation/retirement:

      • Follow MTF/VA instructions below.

    • LESS than 90 days prior to separation/retirement: SHPE MUST be completed at MTF.

      • Initiate DD Form 2807-1, Report of Medical History by going to “SHPE” Module and selecting “Start New SHPE” in MyIMR at link: http://asims.imr.health.mil/imr/myimr.aspx

      • Schedule appointments with SHPE coordinator using the org box email above.

  • Steps to complete your SHPE at MTF: Completion must be <180 days prior to separation, terminal, or retirement, whichever is sooner.

    • Initiate DD Form 2807-1, Report of Medical History by going to “SHPE” Module and selecting “Start New SHPE” in MyIMR at link: http://asims.imr.health.mil/imr/myimr.aspx

    • Complete SHPE Mental Health Assessment (MyIMR)

    • Schedule and complete Audiogram (must be within 6 months of separation date)- contact appointment line at 1-866-DRS-APPT (1-866-377-2778)

    • Complete any other Individual Medical Readiness (IMR) requirements not currently green:

      • PHA/MHA

      • HIV lab

      • Occupational Health termination exam (specific AFSC’s only)

    • SHPE Coordinator schedules Face-to-face SHPE appointment and provides Memo. (Bring to appointment for signature)

    • SHPE Coordinator verifies SHPE completion and signs vOP in vMPF.

    • Filing a VA claim? Contact VA benefits advisor at McGuire: 571-461-8702 

  • Steps to completing your SHA (in lieu of SHPE) at the VA: Must be completed greater than 30 days of final out.

    • Contact Military Service Coordinators (MSC) at the AFRC

    • Initiate DD Form 2807-1, Report of Medical History by going to “SHPE” Module and selecting “Start New SHPE” in MyIMR at link: http://asims.imr.health.mil/imr/myimr.aspx

      • Must be completed within 180 days from separation, terminal, or retirement, whichever is sooner.

      • Do not check the “VA SHPE” box (Answer “NO” when prompted), regardless of whether you plan to file a claim or not, the system won’t allow you to complete the MHA portion with this box checked, this has no bearing on any claims made to the VA as that is a separate process.

    • Schedule and complete Audiogram (must be within 6 months of separation date)- contact appointment line at 1-866-DRS-APPT (1-866-377-2778)

    • Complete any other Individual Medical Readiness (IMR) requirements not currently green:

      • PHA/MHA

      • HIV lab

      • Occupational Health termination exam (AD only)

    • Complete the Separation Health Assessment (SHA) completed as part of the VA claims process can fulfill the SHPE requirement if documentation is uploaded in the members electronic health record 30 days prior to separation. If unable to review at least 30 days prior of DOS, a SHPE will need to be completed at MTF.

    • Present to Flight Med for SHPE Coordinator to verify SHA, SHPE memo, and signs vOP in vMPF.
