Medical Records

All beneficiaries are authorized one hard copy of their entire medical record once per year. To request a copy, patients must complete a Department of Defense Form 2870 at the medical records section located on the first floor of the 87th Medical Group. Hard copy requests are fulfilled within 30 calendar days. Patients also have the option to log on to MHS GENESIS to access or print their medical records and clinical notes.
